Reusable question sets for user testing. Attach one when you assign a sample to a tester — the questions are snapshotted onto that test, so editing the questionnaire later never changes a tester's answers.

You can also create a questionnaire in a modal while setting up a new user test; it is saved to this reusable catalog and selected immediately.


What it is

A questionnaire is a named set of up to 10 questions you reuse across user tests. Each test can attach one questionnaire (or run "acknowledgment only" with none).

Question types

  • Short text — a single line
  • Long text (paragraph) — a multi-line answer
  • Dropdown — select one — pick one from a list of options
  • Dropdown — select many — pick several from a list (renders as checkboxes for the tester)
  • Photo upload — the tester attaches images

Set it up: step by step

  1. Go to Settings → Questionnaires.
  2. In New questionnaire, enter a Name (e.g. "Ring wear test") and an optional Description, then click Create questionnaire.
  3. On the new questionnaire card, use Add question:
    • Type the question prompt.
    • Choose a Type and whether it's Required or Optional.
    • For the two dropdown types, fill Dropdown options (one per line).
    • Click Add question.
  4. Reorder questions with the up/down chevrons; edit any field inline (it autosaves); delete with the trash icon.

The card header shows Questions (n/10). At 10 questions, the add form is replaced with "Reached the 10-question maximum."

Good to know

  • Snapshotting. When a tester is assigned a questionnaire, its questions are copied onto that test. Later edits/deletes here won't alter in-flight or completed tests — safe to keep improving your questionnaires.
  • Dropdown options only apply to the two dropdown types — the options field is ignored for text and photo questions.
  • Questionnaires are optional — a user test can run with just the acknowledgment step.
